        The existence of competition between the increasingly stringent product today requires every company to provide the best for consumers. PT. Unison is a company engaged in the production of bolts, with a product metric hexagon bolts, UNC withworth, and steel hexagon with all sorts of sizes. These bolts are made from raw materials purchased wire rod from PT. Krakatau Steel and PT. Ispat Indo, wire rod materials are also imported directly from China and Taiwan. At present the quality of the products in PT. Unison is not maximized, this is indicated by the large number of product defect is large enough. The study focused on product type metric hexagon bolt with greatest number of defects compared to other products in the amount of bolts 4.12%. Products of metric hexagon bolts to specifications beyond standard of quality set by the PT. Unison and categorized the types of disability: chipped head bolts, screw tilted, bent bolts, head bolt and bolt head tilted cracks. Given these problems, the research carried out using DMAIC method. DMAIC method is expected to defect of 4.12% of each output metric hexagon bolts can be minimized in the direction of the principal terms of zero defect and customer needs can be met. With increasing value of the quality of the product, the company will automatically be able to better compete in the market. The results showed that the level of product quality metric hexagon bolts are measured in terms of the value of DPMO and sigma level during January to December 2011, the number of defect inspection as much as 125.520 and 885 is: DPMO = 1410 and the level of sigma = 4,5. Factors - factors affecting these results is due to lack of maintenance and engine lubricants, the operator made a mistake when set up, less cleaning and coating material, the work environment is noisy and stuffy, so as to determine methods of repair used FMEA (Failure Mode and Effect Analysis).
